AffordableHealthCard membership program
(AHC). I agree to pay AHC a one time enrolment and
activation fee of $15 and
pay AHC monthly Membership Fees until I notify AHC
that I wish to cancel my membership. I understand
that this is not an insurance policy and that I am
responsible for paying the providers promptly for
all services received when accessing AHC's networks.

and the specific services provided. I understand that
neither AHC program nor the networks accessed are
responsible for the outcome of the medical care received
